Come along and join us for afternoon tea in Augusta, during Advance Care Planning Week.
Advance care planning involves planning for your future health care and communicating your treatment preferences.
There will be an interactive community education session held on Advance Care Planning and Advance Health Directives, facilitated by Leisa Prangnell and Dr Sandra Rennie. Leisa is a passionate Aged Care Advocate, Nurse and Educator. Sandra is a Doctor and Medical Educator, specialising in emergency medicine.
The Australian Red Cross will also be present to share information about their services available to communities in the South West.
Tea, coffee and afternoon tea provided.
